App doesn't stop playing

I am listening on my iPhone. I pause a video or audiobook. The app sometimes continues playing after I press pause. A similar thing happens if I have multiple browsers open on the computer - I never pressed play, but randomly it decides to play anyway. I find that I have to close the phone app completely (swiping up from the home screen) to prevent the chance of that happening.
